Private Stretching Class 
Trainer-Client Agreement

This agreement is made on May 6th, 2024 and made between Amanda Boike Fitness/Beyond Human LLC, "the Trainer", and Elaine Pinacate, “the Client”. The purpose of this document is to define Trainer and Client expectations and is effective for the duration of the working relationship between the Trainer and the Client.

Event Date: Saturday, May 11th, 2024
Event Time: 11am-12pm (Trainer arrival by 10:45am)
Event Location: 4912 W Melrose Chicago, IL 60641

Event Details:
The Trainer will lead a 60-minute stretching class for 5-6 guests of "Sip & Stretch," a gathering of friends celebrating accomplishments, transitions, birthdays, and wellness.  

Equipment:
The Client will provide mats, playlist and speaker for class (if desired). The Trainer will provide their own mat for movement demonstration. 

Cancellation Policy
We understand that life happens, and we maintain a cancelation policy that allows as much flexibility as possible.  To cancel a booked event, the Client must notify the Trainer either in-person or by phone, text or email. In the event of cancellation by the Trainer, the Trainer is responsible for notifying the Client of any such cancellation. 

If cancelation is made by the Client one week or less prior to the event (on or after May 4th, 2024) Trainer and Client agree to cancellable costs of 50% the service base rate ($65.00). For private events requiring a deposit, no additional fee will be charged for cancelation.


The Client is expected to communicate late arrival as soon as possible.

In the event the Trainer is more than 10 minutes late on arrival to the session, the Client may have the remaining time of the session added to that session, if possible.


Payment
The total costs for 60-minute service are as follows: Base rate of $130 ($20 travel fee included) + $15.00/per participant

A deposit of 50% the base rate ($65.00 for 60-minute services) is required to reserve your event date and time, with the remaining balance due the day of the event upon completion of the service. 

Payments may be made by credit card using the emailed invoice or via Zelle using the information below.
